DOT CDL Exams, SSRI Medications, and FMCSA Regulations: What Drivers Need to Know

Commercial drivers often have questions and concerns about how medications may affect their DOT medical certification; especially when it comes to antidepressants such as SSRIs. The good news is that being prescribed an SSRI does not automatically disqualify a driver from passing a DOT CDL exam. Understanding how the regulations work can help drivers feel prepared and confident going into their exam.

SSRIs and DOT Medical Certification

Selective Serotonin Reuptake Inhibitors (SSRIs) are commonly prescribed medications used to treat conditions such as depression and anxiety. The FMCSA (Federal Motor Carrier Safety Administration) does not maintain a list of prohibited medications. Instead, medical examiners evaluate each driver on a case-by-case basis.

Examples of SSRI medications are: Prozac, Zoloft, Lexapro, Celexa and Paxil.

Drivers may take SSRI medications and still be medically qualified as long as:

The medication is safe and effective for the driver

The condition being treated is stable and well-controlled

The driver does not experience side effects that could impair safe driving, such as excessive drowsiness, dizziness, slowed reaction time, or impaired judgment

The driver can safely perform the duties of a commercial motor vehicle operator

If these criteria are met, drivers are often certified for one year at a time, which is typical when ongoing medication monitoring is involved.

What to Expect During a DOT Exam

During a DOT physical, the certified medical examiner will review the driver’s medical history, medications, and overall health. If a driver is taking an SSRI, the examiner may ask:

How long the driver has been on the medication

Whether the dosage is stable

If there are any side effects

Whether the prescribing provider considers the driver safe to operate a commercial vehicle

The goal of the exam is not to disqualify drivers unnecessarily, but to ensure public safety and driver wellness.

Taking an SSRI medication does not mean your driving career is at risk. As long as the medication is working well, free of impairing side effects, and your condition is stable, you can typically be cleared to operate a commercial vehicle for a one-year certification period.
